contains a "Medical Release" for club members. Clubs should keep the original copy that the parent
completes and signs. (Clubs should also keep a copy of the birth certificate with each medical release.)

Instructions for Clubs and their Members for Individual YES-Athletics Memberships:
Your club must be an approved YЄS-Athletics club/team BEFORE members of your club/team can apply for
their individual YЄS-Athletics Memberships. (For information, click on "Club/Team Memberships" below.)
The YЄS-Athletics membership forms for the members of your club/team must be submitted to YЄS-Athletics
by your club/team. Your club/team should make a copy of each individual membership form (from your club)
and mail the copy (front only) to Youth ЄNDURO Sports, Inc. for membership processing. Your club members
must make their payment for their individual memberships to your club or the person designated by your club.
Your club/team should then make payment to YЄS-Athletics for those individual YЄS-Athletics memberships.
If you are a member of a club/team, you will receive your YЄS-Athletics ID (membership) Number from your club.
If you are a member of a club/team, Please do NOT submit your individual membership form to YЄS-Athletics.
Your individual YЄS-Athletics membership form can ONLY be submitted to YЄS-Athletics by your club/team.
You must also make the payment for your individual membership payable to your club/team or to the person
designated by your club/team. (Membership fees received directly from club/team members will be returned by YЄS-Athletics.)
This process allows club members to obtain a YЄS-Athletics Individual Membership locally and immediately.
If your club does not have enough athletes to compete as a team in one or more of the age groups, your club's
athletes in those age groups should still compete as a member of your club - not as an "unattached" athlete.

Important Notice for coaches and club volunteers: If the members of your youth club are
members of YЄS-Athletics, you (as a coach or club volunteer) do not have to have an "adult"
membership to be covered by your club's practice and competition liability insurance. You
are automatically covered at no additional cost - a benefit of YЄS-Athletics. Therefore,
we do not offer "Adult Memberships" because they are not needed for YES-Athletics.
If adults want to compete in a YES-Athletics event, they may do so by paying the
participation fee for that event and completing and signing the requires waiver.

Instructions for "Unattached" athletes for Individual YES-Athletics Memberships:
Please mail your YЄS-Athletics Individual Membership form & membership fee directly to the address below.
Please make check (money order) payable to: YЄS-Athletics. You will receive confirmation, by email, that
your YЄS-Athletics Individual Membership has been received and processed. (Allow 24-48 hours for confirmation.)
Your YЄS-Athletics I.D. Number will be included in the confirmation email. (Print the email and keep it for your records.)

CCCNYC Registration must be received by the deadline: December 1, 2009.
Attention: If any part of a Membership Form is unreadable, it must be returned to you unprocessed.
If you are entering the CCCNYC Meet "unattached" and need assistance, you may also call the
YЄS-Athletics national phone number (405-241-6006), If there is no answer, please leave
a message. Calls are returned the same day they are received, up to 10pm central
time. If you leave a message, please repeat your phone number TWICE
very clearly in your message so it can be easily understood.
